DEATH CERTIFICATES

If the death occurred in Montpelier, or the deceased resided in Montpelier and the death occurred within the state of Vermont, you may obtain a death certificate from the City Clerk's Office at:

Otherwise, you may obtain a death certificate from the State Public Records Division.

You may obtain a copy of a death certificate from the City Clerk's office in two ways:

1. by mailing a request to the Clerk's office, or
2. by physically entering the office (at 39 Main Street, Montpelier).


The cost for a death certificate is $10.00.

A request by mail will be processed and sent out the day it is received at the Clerk's office. You must make sure of three things:

1. Your check or money order should be made payable to: City of Montpelier.
2. Enclose the payment along with your request.
3. State the full name and date of death on the request.
